oh dats my bad @Radhesh Sirohiya UPSC ki website par hai kahin attestation form..that is not a big deal…I think last year se self attestation ho gaya hai…it contains some basic details like a last 5-6 year addresses where one has stayed…native addresses..two people who can be contacted for police verification etc

when?? -- i don't remember exactly but around time when interview starts
