queries on summon letter

Could some one please help me with the following queries:
1) regarding acknowledgement of the receipt of the letter? how do we do it
2) my metric certificate doesnt have my full surname (eg. XXXXXX . Y), but my degree certificates have full name (XXXXXX YYYYYY). is this ok? or should be produce an affadavit?
3) some of the spaces in the attestation form are too small (eg. family details and addresses) do we have to fit everything in the prescribed space or prepare a separate document with increased spaces
4) Iam not taking train to travel to Delhi. Do we still need to fill in TA form

Which is the last date of interviews. Mines 28th April. One of my friends 2nd may. Anyone beyond this date?

1. U can mail to upsc..bt even u don't,nothing is going to happen.
2. Yes,there is a requirement of affidavit
3. You have to fill all details in that provided space
4. You will have not to produce a ta,if you don't travel
