Attempted all questions and was expecting something around 280 as last year despite not so good attempt scored 247. Optional requires no specialised prep as most of the syllabus is very regular in nature and simultaneously preparing along with medical pg books would add quality to answers. Diagrams and flowcharts are a must. I would say it is an advisable optional as history doesn't seem to pay you off well.
