    4. A general lack of civic cleanliness sense :
open dumps in cities and towns
open slaughter houses , butcher shops , fish market with little health safety regulation
    5. Infrastr inadequacy :hospital management , waste disposal , drainage sys , etc
    6. other : little town planning , insensitivity of med professionals(?) as well as govt officials
